The word is "Scots" and it will depend on the Scotsman. In theory you should but some don't wear anything underneath as a kilt is a very warm thing. A properly made one is made of 8 yards of pure wool and that's a LOT of fabric when you think your waist is only about a yard around.. The rest is taken up with very deep pleats at the back and a double thickness at the front - it's not fully sewn up and you overlap the two ends at the front to create that double thickness. I know, I've worn one, and on a warm day it gets sweaty underneath.
You're not likely to see many men in kilts anyway. These days they tend to only appear at weddings and other formal occasions.
I just wonder what sort of a person it is whose main thought about someone wearing clothes is what they've got on underneath.

If you ask a Scotsman if anything is worn under his kilt, the standard reply is "No. It's in perfect working order."

Well I wear the kilt and nothing under - most refreshing. The vast majority of kilt wearers wear nothing under.
You can only find out the truth of it by personal experience. You should be so lucky to get that close to a Scotsman.
