44 is the country code for the UK, and + means "insert the international access code here". In the US, that is 011. So to dial a British number, you dial 011 44 and the number, leaving off the first 0 .(All British numbers start with 0 because back in the days of mechanical exchanges, they needed that to connect correctly. But dialling internationally, you have to leave it out.)
BUT wherever they are in the world, if they have a US phone, it's still a US phone. So you don't add anything - just dial the same as normal. Your phone knows that - or should know that - so you don't need to do anything. Just use the number you already have in your contacts.

If your friend has a USA registered phone number, and he does not change the SIM or number when he enters the UK, you should still use the US number. Then, if he has roaming activated, the networks will find his phone. The network will automatically add the appropriate country code.
He may be using an international card. Some of the calling cards show up as +44 which is the UK country code. I know someone who calls from the UAE from an international card because the rates are cheaper and the number always showed up as +44 instead of +97.
No, you must use country codes to mean the country where the phone belongs, not the one where the phone is. So +44 says this is a UK phone, and you will get through to it even if it is in Italy or Greece or USA.
44 is for UK....+ means dial whatever for international...then 44 for UK and the number MINUS the ZERO at start......To dial someone on their mobile you dial what would dial if they are in their OWN country.....NOT code of where they are!!!! Sounds like they have got a local SIM...I did same in NZ last month.
