Are You sure that Ukraine should ask Russia permission about this ?
It is not called the CIS. It is the Russian Federation.
Ukraine did not ratify устав of CIS. One cannot leave something he never entered.

Russia or any other member of CIS has no rights to "allow" or "prohibit" any other country to leave CIS. And you know it perfectly. See sample of Georgia.
