If you are looking to design just one shirt, you might want to look at LogoSportswear's T-Shirt Trial. It was $5 to print a white t-shirt. It's $5 more if you want to use a black t-shirt. If you are looking to sell these shirts, you can build a store on their site for free and upload your designs onto tons of products.
